Cancer Survivorship Defined, Identification of Patient Needs at Survivorship Visit, and Commission on Cancer’s Requirements for Survivorship Care Plans

These modules describe the survivorship stage of care, how to conduct a screening for patient’s needs as they transition to the survivorship stage of care, and identifies the requirements for The Commission on Cancer’s Standard 3.3 Survivorship Care Plan.

Target Audience

This educational program is designed to meet the educational needs of oncologists, nurses, physician assistants, social workers, and other health care/oncology professionals who manage patients with cancer.

Learning Objectives

Following this activity, participants should be able to:

  • Define who is a cancer survivor
  • Describe what is survivorship care
  • Discuss trends of the survivorship population
  • Explain which guidelines and instruments form the basis for the various sections of the Survivorship Appointment Tool
  • Describe the appropriate timing for the administration of the Survivorship Appointment Tool
  • Discuss examples of how the tool is being used and its impact on the survivorship appointment visit
  • Identify the minimal data elements required by CoC Program Standard 3.3 to be included in cancer survivorship care plans
  • Describe the process requirements of a survivorship care plan
  • Summarize the timeline by which CoC-accredited institutions need to provide comprehensive care summaries and follow-up plans
