Transition of Care: Homebound Hematopoietic Cell Transplantation

Hematopoietic cell transplant (HCT) in an outpatient setting requires well-defined coordinated care by a dedicated team including transplant physician, oncology nurse with expertise in caring for patients receiving HCT, and a competent full-time caregiver. Comprehensive psychosocial assessment (to ensure necessary resources are in place) and early implementation of supportive care services are critical for the successful long-term management of patients undergoing HCT. Educating physicians and oncology nurses about the requirements and advantages of an outpatient HCT program would assist in the implementation of an outpatient HCT program in their institution and the identification of appropriate patients who could benefit from this treatment approach.
