Advances in Oncofertility: Fertility Preservation Options for Adolescents and Young Adults with Cancer

Numerous barriers exist to fertility preservation among adolescent and young adults (AYA) individuals with cancer, including excess cost and lack of insurance coverage as well as poor knowledge among providers. By way of a standardized risk stratification model, providers can estimate the gonadotoxic risks of cancer therapy and discuss the available fertility preservation options. While options for females include mature oocyte, embryo, or ovarian tissue cryopreservation, options for males include sperm or testicular tissue cryopreservation. Given that the AYA age range of 15-39 years includes the primary reproductive years, the gonadotoxic effects of cancer treatment and the existing fertility preservation methods must be addressed by a trained multidisciplinary team of providers. Education regarding oncofertility is required to bridge the gap in AYA cancer care and improve outcomes in this population.

Target Audience

The program is designed to meet the needs and improve the competence and performance of the interprofessional oncology care team, including physicians, nurse practitioners, nurses, physician assistants, pharmacists, and other relevant health care professionals who manage the care of patients with cancer.

Learning Objectives

Following this program, participants should be able to:

  • Recognize the barriers to fertility preservation in AYAs with cancer, including lack of adequate knowledge among providers.
  • Assess the risks of gonadotoxicity/gonadal insufficiency associated with various anti-neoplastic regimens, as estimated by the standardized risk stratification model proposed by the Oncofertility Consortium.
  • Discuss the various fertility preservation methods and alternatives currently available for female and male AYAs.
